Mark Zilberman, Director of NorthStar Care and Guidance, LLC provides private care management services and psychotherapeutic counseling for seniors and disabled adults throughout the metropolitan New York and New Jersey region.

NorthStar Care and Guidance is designed to help older and disabled adults and their families to manage the complexities of caring while still living YOUR life. We seek to tailor a plan that will allow you to be as involved as you want to be.

NorthStar seeks to protect you with zealous and vigorous advocacy. At a time when you may feel overwhelmed, we will seek to protect your interests. We are social workers, firstly.

We will save you money. We've done this before. We know what's needed and what's not. It will be done right the first time.

Services:

  • Assess the needs of the person. Will make recommendations for solutions to problems identified. Also, will identify previously unknown problems and risks.
  • Arrange for in home care.
  • Assist in alternative living arrangements.
  • Monitor and advocate for the person once they're placed in the new residence.
  • Perform on-going home visits.
  • Manage medical care (ie., appointments, medication filling, etc...)
  • Accompany to medical appointments.
  • Assist in plan changes as conditions change.
