Connie Rosenberg, President of Services and Resources for Seniors, Inc., has provided private care management services for seniors and disabled adults throughout Northern New Jersey since 1987.
Services and Resources for Seniors (SRS) is designed to help older and disabled adults and their families to develop specific plans of care. SRS focuses on the client and family and seeks to blend the loving care of the family network with the available resources in the community.
Together we will consider the concerns of the family and the limitations of the older/disabled person to develop and help to implement practical solutions for problems of daily living.

Services:
- Assess the needs of the older person and make recommendations for solutions to address the identified problems e.g. adult day care, home safety features, social activities, and other community resources
- Arrange for home care personnel
- Provide assistance with placement in assisted living facilities or nursing homes
- Monitor the care of your family member in a nursing home or an assisted living.
- Provide on-going care management services which may include:
- Visits as desired by the client or family
- Assist with monitoring medications
- Coordinate medical appointments and medical information
- Transportation to medical appointments
- Find appropriate solutions before crisis occurs
- Assist families in positive decision making
- Develop long range plans for older loved ones
