General Comments:
Carolina Eldercare, Inc. was formed in 2003 in response to the growing need for care management services for older adults. As the number of elderly citizens in America continues to grow, so too do the needs for support services. Advancing age is often accompanied by declining health and mental faculties and this is a time when support services can be of great value.
Carolina Eldercare is uniquely capable of supplying these support services. With a trained network of care managers, Carolina Eldercare is available to support the elder client and their family through careful needs analysis, care plan design, plan implementation and a range of other services custom designed to meet the needs of the individual.
Geriatric Care Management
With training in gerontology, social work, nursing or counseling, and - most importantly - a sincere desire to assist the aged population, the elder care manager is a professional who specializes in assisting older people and their families with long term care arrangements.
Care managers conduct care-planning assessment to identify problems, determine eligibility for assistance and assess the need for services. Crisis intervention, counseling and support are among the skills of the geriatric care manager (GCM).
