General Comments:
I have over twenty years in the health care field working with the elderly. I have worked in convalescent homes, hospitals, Assisted Living settings and independent residential care settings. I have been responsible for social services, therapeutic recreational services, volunteer programs, orientation and teaching of all staff, and spiritual care. The Assisted Living setting I managed received a 98% rating from JCAHO.
I graduated from the University of the State of New York and was a public school teacher before attending Yale Divinity School where I focused on medical ethics and pastoral counseling. I received my clinical pastoral education at Yale New Haven Hospital and Masonic Health Care Center. I trained and worked as a pastoral counselor at Hospital of St. Raphael. Bernadette also works part time for a non-medical senior support system, Home Instead Senior Care.
I love to teach and love to work with family systems.

- ASSESSMENTS
Assess current level of functioning. Identify issues and needs, eligibility for assistance, for current and future needs. - CONSULTATIONS
Counseling, psychosocial support, spiritual support, education and advocacy for elders and their families. Identify appropriate resources in community and nationwide. Liaison to families and long distance caregivers - CARE MANAGEMENT
Develop plan of care after needs assessment. Coordinate and monitor in-home or in-facility help and additional health services. Coordinate services of all other professionals. Crisis intervention With release, monitor hospital admissions and advocate for client. - PLACEMENT
Offer guidance in identifying housing options and facilitate the transition. Assist with all placement process. - HOME CARE
Coordinate and monitor all types of home care Re-evaluate regularly. Report to family regularly
Office Policies:
A $150 retainer fee is required to secure services.
Fee Schedule:
$175 for an initial assessment and development of a suggested Care Plan.
Thereafter charges are assessed on an hourly basis.
